Before filling in the form below, access the secure government portal to complete your identity check.
Complete the 5-Minute ID Check
The verification takes roughly 5–10 minutes. You will need a smartphone and a valid UK photo ID (such as a Passport or Photocard Driving Licence).
Copy Your 11-Digit Personal Code
Once verified, Companies House will issue your unique 11-character personal code. Please keep this code safe and paste it into the PSC Identity Code field in our form below so we can complete your company formation.
Director Compliance Declaration
To comply with Companies House identity verification rules, this form must be completed directly by the proposed Company Director (or Primary Director if appointing multiple directors).
